This is a non-urgent service for people seeking advice or signposting for issues such as Long Term Conditions, mental wellbeing, housing problems, debt management and social isolation.
We aim to contact you within 5 days and offer an appointment within 3 weeks.
To access this service you must be over 18 years old and registered with a GP practice in Stockport. The Social Prescribing Team uses an integrated system with your GP Practice. This means your case worker will have access to your medical records as part of the consultation and your GP Practice will be able to access your social prescribing records.
